The eViP Common Consent Form was created for recording patient information based on the similarities bewteen parenting countries. This form has been reviewed by the legal department of each eViP partner institution. Although patient information is not considered Intellectual Property, gaining consent is a vital to the eViP programme.
This document can be used to obtain permission from an individual to use their personal medical information. It complies with national regulations and institutional policies across the EU, and permits use by all eViP partners.
This document establishes who will gain access to the information, the purpose of the study, an explanation of the uses and disclosures and what kind of teaching will be involved.
